Management of Medical Conditions in School

If your student has a chronic health condition that requires support during the school day, a Medication Authorization or an Action Plan needs to be on file so that we can best meet their needs.  Below, you will find plans that can be printed and taken to the provider. If you have a different form of documentation, feel free to turn that in with your signature on the Medication Administration document. 

*Please note: Beginning the 2025-26 school year, all medications INCLUDING over the counter medications will require a physician signature on the Medication Request Form (per policy 5703).  A printed screen shot of the student’s medical portal or a printout of a recent discharge summary is acceptable for over the counter medications.  Please fill out a form for each medication container - only the parent/guardian would need to sign the form in this case.

*All Health Information in Skyward must be updated regularly.  Please be sure to share any current allergies or health conditions of your students with the building Administrative Assistant or contact the District Health Coordinator with any question or concern.
